Expression of Interest (EOI) Form

When users have finished selecting the samples they wish to request, they should proceed to the Expression of Interest (EOI) form. This can be done via selecting “Proceed to EOI” from the Sample Cart, or via the View or Edit options under the Actions menu on the My Sample Requests page.

EOI Form Content

The EOI form asks users for various information designed to assist TERN Adelaide in assessing the applicant's proposed use of the samples. Due to the finite nature of samples held, users are asked to provide information on the following:

  • the institution they are applying from,

  • the person who will act as Specimen Custodian,

  • details of the research/project the samples are being requested for,

  • funding bodies,

  • collaborating organisations, and

  • details of where any loaned samples will be housed whilst on loan from TERN.

The Specimen Custodian is the person responsible for ensuring any usage and/or storage of any loaned specimens/samples is in accordance with the terms and conditions and conditions applicable to the loan. In most instances, for student projects, the supervisor would be expected to be the custodian.

Users should confirm with their institution who should act as the Specimen Custodian for any loan they wish to apply for, before submitting the EOI.

After submission of the form, the TERN Adelaide team will follow up with applicant directly, to clarify any information and progress the request. Users should be aware that the EOI is the first step in the application process, and depending on the nature of the request, sample access may require specific terms and conditions and/or biosecurity or other permits. Any such requirements will be discussed with the applicant after the EOI has been assessed.

 

Filling in the Expression of Interest (EOI) Form

On selection of the “Proceed to EOI” option from either the Sample Cart, View Sample Request or Edit Sample Request pages, the EOI form will generate, prefilling the user's name, organisation and email address in the first section of applicant details.

Users must fill in the remainder of the form, with the majority of fields mandatory for successful submission. These fields are marked with a red asterisk * and will be highlighted red with an error message if left blank. Where relevant, tooltips outlining the information the field requires are present on fields and can be seen beneath the field title.

Each section of the form is collapsable if users click on the sub-heading. Users can then click on the sub-heading a second time to expand sections.

The samples selected are shown at the bottom of the form, along with auto-calculated details of the number of sites/plots these are from.

At the very end of the form, beneath the “Save Draft” and “Submit” buttons, users can see a summary of any fields missing. Expanding this section shows the user what is missing from the form that will prevent it being submitted and provides a link to each of the empty mandatory fields.

Users should ensure all other sections of the form are expanded before using the Missing Fields links to navigate to empty mandatory fields.
